47. A hyperactive 6-year-old girl develops microcytic anemia, cerebral edema, and ataxia. Determination of which of the following serum enzyme activities is most likely to confirm the diagnosis? A) 5-Aminolevulinic acid (δ-ALA) dehydrogenase B) Glucose 6-phosphate dehydrogenase C) Glutathione transferase D) Lactate dehydrogenase E) Pyruvate kinase 50. What is the major consequence of a hereditary defect in the production of active pyruvate kinase in the liver and in erythrocytes? A) Decreased capacity to fix CO2 B) Decreased gluconeogenesis from alanine C) Decreased oxidation of pyruvate D) Decreased production of ATP from glucose E) Increased affinity of hemoglobin for oxygen Please try to explain your answers. thanks.

1.A 2.D because pyruvate kinase is the last enzyme in conversion of glucose to pyruvate, glycolysis, so lack of it means no ATP

47 - is lead poisoning: Lead inhibits delta aminolevulinic acid dehydrase (delta-ALAD) and ferrochelatase (heme-synthetase). As a result, delta-ALAD cannot be converted into porphobilinogen nor can iron be incorporated into the protoporphyrin ring. So the answer is A

answers A AND D. NOT E FOR THE SECOND ONE BECOZ.....PYRUVATE KINASE IS INVOLVED IN SUBSTRATE LEVEL PHOSPHRYLATION in glycolosis AND IF IT DEFICIENT...SURELY THERE WILL BE LESS ATP in the glycolysis cycle.......one more thing PK deficiency increses BIphosphoglycerate so there will be decrease affinity for Hb-O2 combination.....so answer can not be E .
