If someone matches in a preliminary year program on a J1 visa, can he spend some time on research (e.g. 1 year) on the same visa and then re-enter a PGY2 on the very same visa? what about preresidency felloships? Don't they need separate visas? Or can they all be on the same J1??? Appreciate your help

Once you work clinically on a J1 visa then you are subject to the 2-year home residency requriement, and cannot change to another visa class (there are some rare exceptions to this). You can switch from a research J to a clinical J but not the reverse. Pre-residency observerships (no salary, no clinical care) can be done on a B1/B2 visa.
