A CHILD WAS BROUTH TO HOSP WITH FASTING HYPOGLYCEMIC ATTACK....HE HAS GENERALIZED HYPERPIGMENTATION........ELECTROLYTE AND ABG SHOWED A PIC OF HYPOKALEMIC ALKALOSIS...AFTER FURTHUR INVESTIGATION HE WAS CONFIRMED TO HAVE A ADRENAL ENZYME DEFICIENCY....WITH THE LIMITED INFORMATION PROVIDED WHAT IS THE LEAST LIKELY CAUSE? 1--21BETA (OH) DEFICIENCY 2--11 BETA OH DEFICIENXY 3--17ALPHA OH DEFICIENCY 4...17 20 LYASE DEFICIENCY

i think ths was already discussed...the ans they concluded was 17alpha OH deficiency....

i think its 11beta OH def due to presence of hypokalemia.

The correct answer is 4 ,17- 20 deficiency is the least likely cause because in this condition cortisole is normal ,so no excess of ACTH which is causing hyperpigmentation in this case.

woh.. least likely, i dint see it. then 21beta OH def can be the ans there is decreased MC activity which is causing hypokalemia..
