Could u plz guide me what reason should i give n type of visa should i apply for when i am going for observership n USMLE CS exam? Should i specify that im going for USMLE CS ? or just telling i am going for observership will suffice? Would it cause any problem further when i apply for visa in december for interview if i didnt specify my earlier visit for CS exam?

B1/B2 is fine. You don't need any other visa type. This is a visitor visa. As long as you're not earning money and not clinically seeing patients, this is the correct type of visa. You can use a multi-entry B1/B2 to return for the CS exam.
