CAN SOME TELL ME WHAT IS THE INTIAL AND BEST DIAGNOSTIC TEST FOR PULMONARY EMOBOLISM AND DVT

inital is VQ scan and best is angiography but why uw says that u need to assess CXRY and ABGS first

similarly DVT intial test is compression ultrasonography and best is venography

r there some special requirements to choose one over another

can someone help plz

hi loreal ,

if the V/Q scan is high probability and high or intermediate clinical suspicion for PE ( the patient has risk factors for PE ) then the patient should be treated and one does not need to go for other tests but if the V/Q scan is low probability and there is high clinical suspision for PE then an angiography is a must for determining what further management is to be done.

and i just went thru my text again and it says the first initial test for PE is V/Q scan and the gold standard is angiography. but dont know about your question about abg's and chest x-ray.sorry.

thanks achilles but have u done uw ,sometimes i get confusedwhat to do there i said that pick abg /xray

can some one help us who did uw ..thanks

i picked abg and cxr on this q. i dont remeber the stem now but the clinical picture was such that pulmonary emb. was not very clear dx. so from the options given i thougt cxr and abg would be best as the INITIAL test.

1) if the stem says that chest x-ray and ABG's have been done then the next step is V/Q scan but if

2)the stem does not mention that chest xray and abg's have been done then the next step is to do chest xray and abg's...and

3)if the stem does not say that chest xray and abg's have been done and the question is not the next step but what is the best diagnostic test then the answer is V/Q scan..

but well another point is that chest x-ray is normal in PE in 90% of patients ...

hi loreal , i am still doing uw ( as advised by you ) and now am realising the difference between next best initial step , diagnostic test , best diagnostic test et all...
