ADH significantly reduces free water clearance by the kidney so I think it is ADH defficinecy -Diabetes Insipidus...

Na,k,cl pump is resposible for generation of free water in thick ascending loop..it reabsorb all these substances to generate free water.In the presence of diuretic like loops,it will not be able to perform this function and the loss of water would be obligate...(i dont remeber the amount of water that will move with each ion...)

if they have asked ..what increases obligate water...than i would have been diuretics..Am i right

Yes agreed, diuretic therapy will be assoc. with obligatory water clearance not free water clearance

its the amount of water which is obligated to go out...that is according to Goljan its about 20 units for 1 unit of each electrolyte...that is 1 unit of Na has 20 units of water going out with it. the same for Cl and K . and when Na, K and 2Cl is reabsorbed you make 80 units of free water..water without these ions...hope this helps
