3- 35 y/o M with Hx of sever PUD has been diagnosed/w prolactinoma. There is family Hx of pituitary tumors. Which of the ffg diagnostic tests maybe abnormal & can help in Dx?
A- FBS
B- Serum Ca
C- Serum calcitonin
D- Urinary metanephrin
E- Serum ferritin
F- Thyroid function tests

Please explain.

I guess it's the thyroid function tests-Due to negative feedback from prolactin on hypotalamo-pituitary axis and hence decrease in TRH from hypothalamus.

It didn't paste the figure:Anyways what it says is that there is a stimulation of prolactin secretion by hypothalamic TRH and that when you have prolactinoma,there is a negative feedback and hence decrease in thyroid hormones.

Dont worry natasa...reet also did,nt go thru it well...hey reet...why menIIb???..he dosent have pheochromocytoma and neuromas...Its is MEN type 1.so only thing missing is parathyroid malignancy..so ans should be ..B(remember 3p,s of MEN 1).

MEN TYPE 1...(wermer synd)Pituitary,pancreatic and parathyroid

MEN TYPE 2...(sipple synd) parathyroid,pheochromocytoma,medullary ca.of thyroid)

MEN TYPE 2b..medullar ca,pheochromocytoma and mucosal neuroma..

Am i rightnod

Answer is B- serum Ca.

I agree it has to be MEN I
MEN TYPE 1...Pituitary,pancreatic and parathyroid AND Peptic ulcers... thats how i remember it. (all the P's)

Yes, Prolactinoma can be in MEN I because MEN I includes Pituitary Adenomas.
Prolactin is from Ant. Pituitary.

It can also secrete increased GH.

So to review:
MEN I: 4P's:

1. Pituitary Adenoma: GH or Prolactin
2. Parathyroid: Hyperplasia or Adenomas
3. Pancreas: Adenoma, Islet cell tumor: beta cells secrete Insulin (insulinoma) and grastrinomaWhich can lead to ZE syndrome (increase gastrin causes increased acid in stomach and thus:
4. Peptic Ulcers (which is a complication of the above tumor)

MEN I is related to MEN I gene
