Name a few causes of hyperamylasemia?

itīs produced in salivary glands, fallopian tubs., pancreas, GI tract (esophagus). So, disorders in these organs.

Right...to name a few:

Perforated peptic ulcer

Ruptured aortic aneurysm

Intestinal obstruction

Parotiditis and of course pancreatic disorders.

Here's something which I wasn't aware of though...In acute hospital setting nearly 1/3 of the elevations in amylase are unrelated to the pancreas.

also other causes like acute renal failure, esophageal perforations etc.
The test is not very specific

Miscelaneus Causes.
Ectopic Amylase production by malignancies in the lung, ovary, pancreas and colon,
Pheochromocitoma, thymoma, multiple myeloma(increase Salivary Amylase) Amylase Type-S, and breast cancer (increaseed pancreatic Amylase) Type-P all are miscellaneus causes of hyperamylasemia.
Anorexia nervosa and Bulimia increased Salivary Amylase.
