LONG STANDING RA leads to amyloidosis and restricve heart. even if you then treat with steroids you cant get a dilated heart

COX inhibitors inhibits cyclooxigenase hence no PG and prostacyclin-no endothelial relaxation-coronaries deprived of O2-hipoxia-CHF in longer term course as heart is deprived of Oxygen and can not pump efficiently to meet body demands. In case of RA-yes it should have been restrictive heart disease,but EITHER way(with or without RA and infiltratioon of myocardium) you should have dilated cardiomyopathy as a consequence. You also get a hypertension,Coronary Ischemia as a consequence of using either non selective COX inhibitors such as ibuprofen or selective COX-2 inhibitors(which have mostly been withdrawn from US market-Bextra,Vioxx,celebrex(celecoxib group) etc.People died with CHF mostly or from a heart attack. I HAD THIS DAMN QUESTION IN ONE OF MY PHARMACOLOGY PhD EXAMS,SO THE ABOVE POSTED IS 100 % CORRECT!

now i dont know whether i should base my ans according to NSAIDs role in course of Disease or the corticosteroid induced complication or Go with the RA complication itself DCM is a Diagnosis of Exclusion way down the list but the most common cause of DCM is ischemia i will go with E bcos RA has a relationship with both Constrictive Pericarditis n Restrictive Cardiomyopathy but more with latter Plz give ur reasons with References plz that helps a lot
