I would say:
simple diffusion - alveolar diffusion of O2 or CO2
facilitated diffusion - diffusion of glucose facilitated by insulin
secondary active diffusion - diffusion of glucose as a co-diffusion with Na+ depending on the Na/K ATP Pump
