hi:

as every one know most img begin their residency on j1 visa. later when they finish their residency and want to resume their practice in the states this will be available only through j1 waiver program.

as i have read from many sources j1 waiver positions available are so limited that is only few of the j1 holders will have the opportunity to practice in the US and the rest have to return to their countries for two years before being allowed to practice medicine in the us. but what i notice is that the minority are who return to their countries wheris the majority remain and work in the US. my question is how can they do this and we know the limited waiver positions.

i will be really thankful for any comment.

As far as I know 20 J1 waiver jobs allowed per state per year.Its getting more and more difficult to obtain waiver jobs these days as what my sources tell me.

My opinion is that if you want to go for fellowship, take j1. If you are happy with primary care, go for h1. From what I know, it is far more difficult to get a fellowship on h1 than to get j1waiver. But I think j1 is much more easier to get during residency and h1. These are things that i have learned frompeople I met during my interviews. S please confirm my opinion with more knowledgable people. But nothing is impossible- getting j1 waiver or getting fellowship onh1, If you are planning to get your degree here and go to your own country for practice, I would say that J1 is a good option.

J1 waiver job requirement is now made 5 years ....which in the long run will make finding j1 waiver jobs tougher - as we will have more J1 holders in the pools graduating out of residencies - while the waiver positions are still being held by the previous guys yet to finish 5 years...

but yes - the golden rule is - if u want good fellowships - dont mind a J1 visa...

and now its 30 waiver jobs per state

Dear digitaldoc:infact 20% of the US population are living in underreserved areas. that are the areas in which j1 holders can have a waiver position to work in. so even extending the period required to be spend in the waiver job to 5 years doesn't mean that all the waiver jobs will be occupied .because the main issue here is to find the employer who is ready to employ j1 holder through j1 waiver program. waht i meant in my question is : i see that MOST of doctors who train in the US remain in the US despite all the talk about the limited number of j1 waiver positions (30 per state) . if it is true that j1 waiver positions is so limited then how can these large number of img remain in the US after finishing their residency???

I have been working with IM residents since 1988. Since that time, I have known approximately 15 who have returned home. Only a handful of them returned because they couldn't find a waiver that they wanted to take.

The positions are out there.

Yes its true that if you want fellowship then J1 visa type is better, however I have been told that its not easy to get J1 waiver jobs after fellowship......but its easier if you're looking for it after residency.

Really, its now 5 years in underserved areas for J1 waiver jobs???? Are you sure?? I thought they were only plans to change the legislation but didn't know its already a new ruling......how can I confirm this???

OK thanks.Anyhow I couldn't find it as well but in the process of searching for it found that some of the states block the J1 waiver jobs for PCPs only(primary care physicians) and do not allow post- fellowship to do J1 waiver job.
