20 yo white woman brought to the ER 3 hours after she took 20 acetaminophen 500mg tablets. Roommate says she drank alcohol for the first time along with seafood last night. She has a 5 pack year history of smoking and is otherwise healthy. She is alert and oriented. Which of the following decreases her risk for hepatic injury? yes alcohol is right. UW says acute alcohol competes with acetaminophen for cytochrome P2E or some such one and thefore the toxic metabolites of acetaminophen are not produced as much. Chronic alcohol however would increase the risk since it induces cytochrome P450 system increasing the metabolism of acetaminophen. wow u guys are good...i had no idea!!!
