aspirin decreases PGE2 and Ag II is unopposed so causes renal papillary necrosis...how is AgII unopposed?

prostaglandins maintain the renal flow by dilating the arterioles, therefore as aspirin reduces the flow , angiotensin II comes into play in order to maintain the GFR, but again aspirin by itself is insufficient to cause renal papillary necrosis, however on top of it if u throw the free radical injury to the papilla induced by acetaminophen, tht's actually a killer combination and over time papillary necrosis occurs.

PGs are vasodilator of afferent arterioles-----inhibited by NSAIDs

angiotensin 2 are vasoconstrictor of efferent arterioles

thus in presence of NSAIDs both aff &eff are constricted
