How do you differentiate when a patient has just Wernike's vs. when he has Wernike-Korsakoff?
(What symptoms are present in W-K, that are not present in W. only?)

They are both connected with alcoholism and they are both treated with theiamine.

However, Korsakoff has a major negative impact on the memory of the patient ( they create imaginery experiences to fill the gaps that they don't really remember ) whereas Wenicke doesn't has a great impact on the memory...Wernicke is mainly connected with confusion and opthalmoplegia...

Korsakoff is characterized by confabulation which is lack of memory early sign. Wernickes is more complex neurological symptoms and it is caused by Thiamine deficiency in the diet and by alcoholism. Characterized ny ataxia, confusion, drowsy, stuporous, nystagmus, and ophtalmoplegia are some of the characteristics.

In what other conditions do we see Thiamine deficiency that can cause Wernicke's?

excessive vomiting during pregnancy, recurrent dyalisis, food faddism, diet deficiency.

good info guys, wernicke... ataxia and ophthalmoplegia

korsakoff, forgetfulnerss and hence confabulation, which means making up stories to cover up...

thiamine def can lead to wet and dry beri beri

To add:
Giving thiamine will reverse the Wernike, but not the Korsakoff.

In addition,in terms of lesion sites,Wernikie and korsakoff primarily effect the dorsomedial nucleas of the thalamus and the mamillary body(hypothalamus) in addtion to other sites like hippocampes and the vermis of the cerebellum.

note:korsakoff causes antegrade and ertrograde amnesia with confabulations.u have probably covered everything else there is to know about these cases.

would someone like to explain the difference between dry and wet beri beri?

what causes each?

wet beriberi---congestive cardiomyopathywith lt/rt HF

DRY beriberi-----demyelination,peripheral neuropathy

both related to ATP deficiency

Yes, wet beri beri is a high output cardiac failure
and dry beri beri is a neuropathy.
(Both due to B1 deficiency).
