A 17 yr old girl presents with sudden onset of abdominal pain. PE reveals a tender mass in the left adnexa. Pregnancy test is negative. A x-ray exhibits a mass lesion of the left ovary with focal areas of calcification. Which of the following best characterizes the ovarian mass?

a. malignant surface derived tumor
b. benign surface derived tumor
c. malignant germ cell tumor
d. follicular cyst of the ovary
e. benign germ cell tumor

its cystic teratoma... ... seen in adolescents, x ray shows calcification..

so e

good explanation roopashri.

sky high just we can do exclusion if we dont know the exact diagnosis. surfal tumour doesnt generally calcify as it is not of mesoderm.and malignancies dont go for calcification.

genrally tumour that is big enough give symptoms is benign and has got torsion.

ok i have a question here why not psammoma bodies as seen in cystadenocarcinoma of the ovary, they re also areas of dystrophic calcification?????

Good point Sturge...

With the benefit of hind-sight wink I'd choose E...Cystic teratoma (young female, unilateral mass, areas of calcification, sudden onset acute abd pain indicating torsion n' infarction)

Serous cystadenoCa does have Psamoma bodies which is dystrophic calcification ubt I think its is older females, bilaeral, and presents with Ascites n' increased abd girth)

What say folks?

well age makes me think and yes bilaterallity, as our great Dr. Goljan says

so may be, but if it were a middle aged female... we would be in big troublewink

older age - serous cyst adeno ca.
