A 31 yr.old type 1 insulin dependent diabetic has a burning sensation around his ankles and the bottom of his feet. PE reveals depressed Achilles and knee jerk deep tendon reflexes bilaterally and decreased light touch sensation in both lower extremties. the mechanism for this disorder is most likely associated with

a. thiamine def.
b. lumbar disk disease
c. pernicious anemia
d. osmotic damage
e. syringomyelia

just wondering....if option A was cyanocobalamin.....wud tht also qualify as an answer?

I don't think it would fit Drk...correct me if I am wrong but I think Vit B12 knocks off the dorsal column and the lateral corticospinal tracts....so yes, you'd see a depressed vibratory sensation etc but I'd think the tendon reflexes would be hyperactive n' not underactive...

he has diabetes, and diabetes causes peripheral neuropathy and the damga it does is by osmotic damage, stock and glove type neuropathy
