Its Acute Bacterial Endocarditis due to Staph Aureus, likely affecting the tricuspid valve with vegetations

leading to

a. Septic Emboli (couging of blood and pleuritic chest pain)

b. vessel inflammation (Petechiae and Hemmorhage)

Hint was acute onset and IV drug abusergrin
