This is a Q that someone told me: A woman comes in to the office saying that her husband cheated on her, and the other woman is experiencing symptoms but the husband is not. The patient is having an "abnormal discharge". She also has nausea/vomiting/abdom pain. Q1-What tests do you run? Q2-What do you do? Start her on AB's before test results come back? if so which AB? OR Wait for Test results and then start on AB? Q3- Any idea of the diagnosis? Chlamydia?

Do a pap smear. Draw labs for GC, Chlamydia, HIV, RPR. Do a culture of the discharge. Get a UA and a CBC w/diff. I think as long as she's having "abnormal discharge" then she should be given Abx before cultures come back. Quinolone and Metro?

why do you do ua? what if the discharge inerferes with the UA?

First do HCG to confirm pregnancy or otherwise. then culture and while waiting for results can start Rx. but the symptoms described are too vague...something more about the discharge and lesions will be given to decide treatment.
