What is the difference between Communicating and NonCommunicating Hydrocephalus?

Communicating means there is connection between the Subarachnoid space and ventricles, but this is normal, correct? Also its due to inc. production or dec. absorption. correct?

Non communicating there is an obstruction so no communication? correct?

What about the symptoms? and presentation?

Do they both have dilated ventricles?

Which one is more severe?

I think there is a distinction between communicating, non-communicating and normal pressure hydrocephalus.


-communicating HC occurs due to obstruciton in the subarachoid space, and can result from overproduction of CSF, defective absorption, or insufficient venous drainage.

-noncommunicating HC occurs when obstruction is in the ventricles.
mostly caused by stroke or traumatic injury to brain

-Normal pressure hydrocephalus i(usually in elderly or <60)s described as increase in ventricular size, and normal CSF pressure, and an absence of papilledema when detected by lumbar puncture. However, patients can have hypertension and a triad of gait apraxia, incontinence and dementia




Hydrocephalus sometimes caused by neural tube defects or bacterial infection, leads to mental retardation in children.

ya they have incressdd pressure in both. nothing is more sever or less. it depends on the progressin of the diseases.

you also have normal pressure hydrocephalus

causes of communicating:
over prod: choroid plexus papilloma
dec abspn(more commonly): arachnoid granulation scarring by meningitis or arachnoid Hge.

Causes of noncommunicating:
MCC in newborns:- aqueductal stenosis
Congenital malformations: Spina bifida, Arnold-chiari, Dandy-walker, agenesis of foramen of Munro, also congenital toxoplasmosis.
Acquired: medulloblastoma, astrocytoma, cysts, abscesses, hematoma, cysticercosis...any mass lesions.


Causes of NPH(normal pressure) [emedicine]
SAH
Head trauma
Meningitis
Tumors
Posterior fossa surgery
Idiopathic, probably related to a deficiency of arachnoid granulations

Does Parinaud's Syndrome (tumor of the pineal gland that compresses the cerebral aqueduct) produce a communicating or noncommunicating hyrrocephalus?

non communicating it blocks the out flow of CSF from 3rd to 4th ventricle

-What is the most common cause of hydrocephalus in a newborn? And can you tell by physical exam if its communicating or noncommunicating?

-What is 'hydrocephalus ex vacuo'? -It occurs in Alzheimers disease.
