hernia tion of the uncus ofthe parahppocampal gyrus of the medial temporal lobe across the tentorium cerebelli. causess 3rd nerve palsy and posterior ciclation defects. due to tumour in supratentorium

subfalcine iswhen cerebral hemisphere is pushed to the other side of the falx cerebri

presents with headache vomitimg. and anterior cerebral artery compression that is leg paresis on either side. urinary incontinence,frontal olbe signs etc



tonsillar herniation --cerebellar tonsil from formen magnum. cause ICT. symptoms -- lhermitte phenomenon

ok a last q, in arnold chiari malformation type 2, do we have a tonsillar herniation

yes we have tonsillar herniation in both

In the
Chiari type 1 malformation, the cerebellar tonsils protrude through the foramen magnum and into the cervical spinal canal; when this abnormality is associated with protrusion of meninges (meningocele) or meninges and cord (meningomyelocele) through a spinal canal that has incompletely closed, it is designated a Chiari type 2 (or Arnold-Chiari) malformat

uncal hern---

oculomotor (CNN III) paresis Ipsilateral pupil dilatation occurs as the parasympathetic fibers, which are located around the outer aspect of the third nerve, are compressed by the uncus. This leads to dysfunction of the parasympathetic fibers with subsequent unopposed sympathetic responds. This will dilate the ipsilateral pupil.

Contralateral hemiparesis occurs with compression of the ipsilateral cerebral peduncle. Since the cortical spinal tracts decussate (cross over) below the mid brain in the level of pons, the hemiparesis is contralateral.

if u have tumor on left side ------left occulomotor nerve is stretched -----dilated pupil on left side

left posterior cerebral artery is compressed----hemianopia on rt side

but rt crus cerebri is damaged (kernoham notct )-------left hemiparesis


KERNOHAN NOTCH GIVE A FALSE LOCALISING SIGN
