It's Extreme obesity with polycythemia, somnolence, hypoventilation, arterial unsaturation and hypercapnia, and pulmonary hypertension.

That's all I know about it.Can lead to cor pulmonale and right heart failure at the end. There's hypoxia and hypercapnia and that's why you get polycythemia as well as vasoconstriction of pulmo arteries-giving pulmo hypertension.

The Pickwickian syndrome, also known as obesity hypoventilation syndrome, is the combination of severe obesity, suffering from OBSTRUCTIVE SLEEP APNEA causing hypoxia and hypercapnia resulting in marked day time somnolennce

and ya when there is obstructive sleep apnea, due to obstruction in the airways, there re apnic episodes and this leads to hypoventilation, as person stops breathing while sleeping due to the obstruction, and when he the hypoxia wakes him up from the sleep, and this leads to morning time hypersomnolence..... however the condition can be treated by positive pressure cpap therapy.

this is a cause of obstructive sleep apnea, which is however different from central sleep apnea...
