What does it say about a program that hasn't filled multiple seats in last yrs match? Is it a bad program or did the program just not rank effectively?

If you have a chance of matching with such a program, I wouldn't worry much. It usually says that PD or hiring committee did not do a good job. Typical problem for the programs is to get a right blend/proportion of candidates to fill interview slots. Too many non-competitive candidates -> program quality goes down. Too many highly competitive candidates -> unfilled positions -> Scramble -> program quality goes down. From applicant's perspective this information can be used only to estimate how likely the program will have any unfilled positions this year, nothing else.
