it is absent pulse in inspiration due to a very low (more negetive) intrathoracis pressure.or fall in bp >10mm hg

seen in emphysema
sever asthma


and due to compression of lv in pericardial diseases during insp
seen in pericadial effusin

tamponade
etc

ok one more q,

what is the other sign seen along with pulsus paradoxus in pericardial effusion...

another q how do u differentiate pericardial effusion and constrictive pericarditis

The other sign would be a raised JVP.A pericardial friction rub would also be heard.

No idea about this one.Although,i think there is mostly an element of effusion with constricitive pericarditis.

In constrictive pericarditis, the central venous and right and left atrial pressure pulses display an M-shaped contour, with prominent x and y descents

well for pericardial effusion we get the pulsus paradoxus and the kussumals sign

to differentiate them, there is a pericardial knock present in constrictive pericarditis

well i didnt know about the m shaped contour... of constrictive pericarditis

Here are some causes of Pulsus Paradoxus (copied)

Cardiac causes
1. Cardiac tamponade
2. Pericardial effusion
3. Constrictive pericarditis
4. Restrictive cardiomyopathy[4]
5. Pulmonary embolism
6. Acute myocardial infarction
7. Cardiogenic shock
Extracardiac pulmonary causes
1. Bronchial asthma
2. Tension pneumothorax
Extracardiac non-pulmonary causes
1. Anaphylactic shock (during urokinase administration)[5]
2. Volvulus of the stomach[6]
3. Diaphragmatic hernia[6]
4. Superior vena cava obstruction[4]
5. Extreme obesity
