The patient has secondary hyperthyroidism (T4 increased, atrial fibrillation). increased T4 but TSH also is increased---> secondary hyperthyroidism--> CT to look for causes in pineal/pituitary gland

I think its case of sick euthyroid syndrome as pt ishaving pneumonia as a result pperipheral conversion of t4 to t3 is not present.I will go with B. ___________________ have fun
