Agree.

but in new kaplan book it is written that a sudden inc in set pt means body temp below regulated core body temp .this will induce mech to conserve heat &raise production.-cut vasocon &shivering ,but body temp will not rise .

96 International Pediatrics/Vol. 20/No. 2/2005 www.int-pediatrics.org
The febrile response is mediated by a group
of low-molecular weight proteins (pyrogens),
which are produced by polymorphonuclear
leukocytes and other macrophages in response
to infections or other insults to body (Fig. 1).
Several mediators act as pyrogens, including
IL-1α, IL-1β, TNFα, IL-6, and IFNα. These
pyrogens stimulate the endothelium in the
hypothalamic vasculature to produce
prostaglandins E2 (PGE2). PGE2 elevates the
set-point of hypothalamic thermostat. A rise
in the hypothalamic set-point triggers the cold
response leading to the thermoregulatory
mechanisms to decrease heat loss (for example
by decreasing blood flow to the skin) and/or
increasing heat production (for example by
shivering) and behavioral changes such as heatseeking
activities. Once the new set-point isreached, heat loss will continue to maintain that
new set temperature. Antipyretics, such as
acetaminophen and ibuprofen decrease the
temperature set-point by inhibiting the
production of prostaglandins which leads to
sweating and vasodilation. This leads to heat
loss and a decrease in body temperature.
