An elderly woman with osteoporosis is taken to the emergency department following a fall. ONe of her legs appears shortened and is externally rotated. A fracture of which part of femur is suggested by these findings?

a. greater trochanter
b. lateral epicondyle
c. medial epicondyle
d. neck
e. shaft

please explain. thanks

neck of the femur. it a wague q but. an elderly osteoporotic will have a very weak neck of the femur. it is not the fracture of greater trochanter because it does not coause shortening. not of the lateral or medial epicondye for the same reason.
If it is shaft and displased it can have shortening but no external rotation.

Also, neck of the femur keeps it internally rotated...

do you mean to say that neck keeps it internally rotated and its fracture leads to external rotation?ok

age also suggests neck of femur.

in actual fact, with a fracture of the shaft fo the femur, the leg may be externally rotated. but that requres a lot of energy/force to fracture. key here is her age,osteoporosis and the low energy fracture. all together answer is neck of femur.

neck - leg shortened & externally rotated
shaft - leg shortened.

explain.:displacement of the lower fragment by the pull of gluteus medius,pyriformis,gamelli,quadratus femoris,hamstrings

* INTERTROCHANTERIC----SAME AS ABOVE BUT IN YOUNGER AND WITH MORE FORCE-see attached file for more differences

*POST. DISLOCATION ----FLEXION,ADD,INTERNAL ROT,SHORT
FADIRS
*ANT DISLOC------FLEX,LENTHENING,ABBDUCTION,EXTERNAL ROTATION
