Sometime MI is silent in older and diabetic ppl . why it is so?

sometimes mi, can also manifest as epigastric pain, and thats mostly when the rt coronary artery involved, but when the left coronary artery involved, the pain radiates to the arm, typical signs

hahahahah, I am his fan so use his picture , he is a wonderful teacher........ Well in case of Diabetes mellitus its because of neuropathy.... but in case of old ppl I am not sure about autonomic neuropathy... any one know???
