In hypovolemic shock, which part of the kidney is affected first by ischemia?

Medulla> cortex...normally receives 10% of the blood flow

Specific sites of ischemic damage: straight segment of PCT and thick ascending segment of loop of Henle (medullary portion)

Please correct me if I am wrong...

yup, thts wht i heard from Goljan too. but i hv also read some conflicting data so i wanted to clarify,

In what conditions is the Cortex(or inner cortex) more prone to ischemia/infarction than the medulla?

if you are talking sickle trait infarctions...i think tht ws also mentioned by Goljan to be medulla.

how abt thromboembolism, for cortex?

Well, for thromoboembolism, when the emboli gets lodged in an artery, it will lead to ischemic changes distal to that segment, which will include cortex as well as medulla.

Is the cortex the part without any collateral blood circulation? if so, is it the most likely to be infarcted first and more severely than the medulla maybe in such cases?
