A patient with long history of hypertention, after a stroke clinical examination shows

Urinary incontinence, the movements of the upper extremities are normal, he can execute a command to move his right arm, but can not execute a common to move his left arm

Urinary incontinence seen with blockage in ACA, mostly bilateral damage of cortex. Inability to execute command to left arm(transcortical apraxia) also a sign of ACA block, damaging the anterior part of corpus callosum. fits with D.

nice puzzle.

lesion of anterior cerebral artery cause transcortical apraxia due to lesion of anterior portion of corpus callosum results in disconnection between the language centre on left side and the motor centre on right side,so the patient can move his right limb as the command comes from left side but can't move his left limb as the command can't cross to the right side to execute the movement.also the anterior cerebral artery supplys the sensory and motor area of the lower limb and pelvis that's why the patient has urinary incontinence.
