A 70-year-old man has severe atherosclerotic disease of the abdominal aorta that obstructs the ostia of the superior and inferior mesenteric arteries. He has intermittent episodes of abdominal pain and bloody diarrhea. Which of the following regions of the bowel is likely to be most severely affected?


A) Ascending colon

B) Descending colon

C) Hepatic flexure

D) Splenic flexure

E) Transverse colon

The splenic flexure is the watershed zone between the superior mesenteric and the inferior mesenteric arteries....which renders this area susceptible to infarction.

The other watershed zone i know of is the distal colon where the watershed area is between the IMA and the hypogastric arteries.

Vascular disease of the bowel Despite its abundant vascular supply, the bowel often suffers damage from lack of blood. Remember that the watershed zones of the large intestine are at the splenic flexure and at the rectosigmoid junction.

I can add that usually the guys with this ischemic colitis are above age of 60 smiling face
