Afterload reduction is an an important therapeutic intervention for most valvular emergencies . Its effect is to optimize forward moving pressure gradients .In AS , however vasodilating agents must be used with caution because they can decrease perfusion pressure across the thickened myocardium . Rate contriol is more important to maximize ventricular out flow and diastolic perfusion time
