Hi, The equation for free water clearance is CH2O=V - C osm per First Aid book. The Princeton book I'm working out of doesn't explain what C osm is and neither does the First Aid. Can someone please tell me what it stands for? Thanks

C osm is clearance of osmotic active substances i.e. plasma volume cleared from osmotically active substances for a period of time

c osm is the obligatory water, that is water that has to be cleared along with the solutes, u calculate it by the same formula as uosm x v/ posm... so it is the water that accompanies the solutes, and adh has no action on it. the free water that is cleared is above this obligatiory water.. and adh can act on decreasing the free water but not the obligatory water
