pt. age 56 yr has headache and been diagnosed HTN. on arteriography right renal artery is stenosed to about 85%. If a surgery is performed what is the best predictor that BP will be normal after operation, when renin level in compared in 1-right renal vein vs left renal vein 2- left renal after vs right renal artery 3- right renal artery vs left renal vein forth I forget plz explain this

1-vein vs. vein.

the point is that Renin is coming out of the kidneys through the veins (like anything else)

well... I guess they are talking about the right kidney synthetizing the renin so it would be found first in the right renal vein and pass through the circulation and then would be found in left renal artery..

stupid thing to say, if you ask me.. this is science fiction

Renal artery stenosis supplies less blood to the kidney and in response renin is secrted which will be found in the renal veins first. That kidney whose artery is stenotic will secrete more renin than the kidney whose artery is normal ( in fact the normal kidney's renin secretion will be suppressed). So measuring renin from the renal veins that is before they get into the systemic circulation allows you to find out which kidney is secreting more, and which is secreting less than normal or normal amount of renin. So the one which is diseased will secrete more renin and the one which s normal will secrete less than or normal amount of renin. So you compare renin secretion from both the renal veins.

actually it is PRA(PLASMA RENIN ACTIVITY).WE ACTUALLY TAKE PLASMA RENIN SAMPLE IN IVC ABOVE AND BELOW RENAL ORFICE AND ALSO FROM RT AND LT RENAL VIENS.PRA IS INCREASED IN VIEN DRAINING THE AFFECTED KIDNEY WHILE IN CONTALATERAL KIDNEY THERE IS DECREASED RENIN ACTIVITY DUE TO SUPPRESSION OF RENIN BY INCREASED ANGIOTENSIN II
