When I read genital gram smears in the micro lab (we read a lot of these as STATs from the ER) the ones that have organisms consistent with Gardnerella (tiny gram variable rods) don't seem to have Lactobacilli on them. Lacto appears as long gram positive rods, distinctly different than GV. That being said, bacterial vaginosis is a complex condition, and Mobiluncus (an anaerobe, which is not cultured for, but appears on the gram) appears to play a part in the syndrome. Some labs use a scoring system for genital gram stains. We only use this system for one of our doctors. http://tinyurl.com/brrox Check out the pdf file above for one hospital's procedure on scoring of gram smears. Ours is similar.
