32. A 2100-g (5 lb 2 oz) male newborn is delivered at 34 weeks' gestation; Apgar scores are 7 and 8 at 1 and 5 minutes, respectively. The 14-year-old mother Priety Chintaa had no prenatal care and did not know how much weight she gained. During the pregnancy, the mother smoked marijuana and took over-the-counter vitamins occasionally; she did not drink alcohol and had no illness except for an upper respiratory tract infection 4 months ago. She did not know she was pregnant until 2 weeks ago; her family is unaware of her condition. She has had one sexual partner. A too Is there a case that the pregnant woman did not know that she has pregnancy until 2 weeks before delivery ?

I don't have the complete answer but A ) Child abuse seems the most likely answer............. Any discrepancies????

Low SE status, other factors - by exclusion A seems most likely.

lbw,preterm baby wont coz seizures. in fact in this case marijuana would be the coz of lbw and preterm baby. and once the preterm baby borns , its exposed to many complications- ards , necro colitis,, etc. but in this case most likely is A. considering young mother , single, drug abuser.

i recollect from Goljan's step1, marijuana effects on fetus are LBW and prematurity,....which in turn inc the incidence of intraventricular hemorrhage leading to seizures. but, after re-reading this Q, i do agree tht thr are far too many signs crying out towards risk of child abuse in this infant. So i wud like to change my answer. (wish USMLE allowed this luxury...;o))
