can someone explain to me as to how ace inhibitors increase survival in a patient with both htn and chf or is it an association? its benefit in diabetes nephropathy is obvious .

An ace (-) will decrease both afterload of the heart and systemic vascular resistance by inhibiting ACE which would have lead to an aldosterone mediated increase in intravascular volume and inhibit angiotensin II mediated vasocontriction.

major problem in heart failure are the compensatory mechanism that become counter productive. One of these mechanisms is activation of renin angiotensin system due to poor renal perfusion. This is counteracted by ACEI, thats why they increase the survival
