What is the mechanism of increased ionized calcium in response to acidosis?

  #2

It is because albumin is the calcium binding protein in serum, acidosis will decrease the negative charge of albumin and decrease its capacity of binding calcium ion. And in case of alkalosis, the negative charge of albumin will increase and albumin binds more calcium ion, patients have the risk of tetany.

you think of it as a competition b/w Ca and H both of which are positive charged to get more spaces on albumin. you increase the H ions and it will displace Ca from some places on albumin.
