Patient is W/N, W/D and in NAD... I guess NAD is for "no apparent distress" ....what are the other two for??

As long as they aren't in the official abbreviation list you cannot use them, so why bother?

Yeah, not in the list. You cannot use them in the real exam! W/D (well developed), W/N (well nourished).
