A man visiting from Indonesia presenting with for fever & sore throat for last 3 days. On examination there was lymphadenopathy, but no splenomegali, lymphophenia, & monospot test was negative. Which of the following you will do?

a) EBV serology

b) Blood culture

c) HIV test

d) Throat swab culture

e) Give him penicillin

definitely not A...Monospot negative...no splenomegaly...I'd start with a throat swab and follow it up with a blood culture....so I'd go with D.

Anyways you'd have to wait till either throat swab and or blood culture comes in before starting Penicillin.

I would suspect HIV.Why throat culture any one can xplain this

Going from above clinical scenario the most likely diagnosis is a simple viral sore throat. I dont think there's any reason to suspect HIV infection from the limited information given. However the only thing that would influence management is a throat swab (i.e if it is streptococcal it would indicate the need for peniciliin)

Ordering HIV tests for a patient with fever and sore throat for 3 days without any other indication of HIV infection seems to be a bit irrational. Anyway I feel this is a poor quality question with probably inaedquate information.
