In a child with occult bacteremia - wd u start empiric rxif the fever was 38 C meanwhile send the BC as well? and hes NOT looking toxic but you've rules out everything else?
And it came back negative, but hes not doing well, wd u continue rx?
(assuming by given details it IS bacteremia?)

If the child is non toxic and fever >39 degrees celcius, take the CBC, if more than 15,000, take culture and give one dose of ceftriaxone or take blood culture and give empirical Rx with ceftraxone.
If culture comes negative, but child looks ill, repeat blood culture, do LP and initiate antibiotic therapy
