I know B1 can cause hypersensitivity, but progressing to death? I am not aware of it.

Very Good...... nod

Parenteral Exposure of thiamine can result into Hypersensitivity reactions like Pruritus, respiratory distress, nausea, abdominal pain, weakness, anxiety, shock and sudden death....

Check for hypersensitivity to thiamine before administration. Facilities for the management of anaphylaxis should be readily available. Toxic reactions have been produced by injection of 50mg of thiamine, and sudden death has been reported after 100mg. Anaphylactic reactions may occur and the risk increases with repeated injections - such reactions are infrequent and unpredictable. An intradermal test dose may be given to patients suspected of thiamine hypersensitivity. Thiamine may enhance the effect of neuromuscular blocking agents.
