A 70-year-old man is admitted to the cardiac care unit following a 30-minute episode of palpitations and associated dizziness. In the emergency department, an electrocardiogram showed monomorphic ventricular tachycardia, and his blood pressure was 90/50 mm Hg. He underwent direct-current (DC) cardioversion and was given a lidocaine bolus (100 mg), and lidocaine infusion was started.
He has a history of coronary artery disease and two prior myocardial infarctions, the most recent 8 weeks ago. A recent echocardiogram showed anterior and inferior wall motion abnormalities and an ejection fraction of 23%. His current medications include aspirin, enalapril, isosorbide mononitrate, furosemide, and simvastatin. He is currently stable, and infarction has been excluded, but three episodes of nonsustained ventricular tachycardia have been noted.
The best long-term therapy for managing this patient’s arrhythmia is:
A) Metoprolol (orally)
B) Amiodarone (orally)
