Why is the Direct Bilirubin not elevated in Primary Biliary Cirrhosis????????????????????

The pathology is obstruction of extrahepatic bile ducts with proliferation and fibosis. I beleive it ( DIRECT Bil) should increased Because it is obstructing the Bile but its not.... As mentioned in USMLE WORLD.............................

hi,

but doesn't primary biliary cirrhosis have auto immune destruction of the 'INTRAHEPATIC BILIARY DUCTS' with the extra hepatic biliary ducts looking normal?

now coming to the increase in direct bilirubin, what i think is that UW is talking of a situation where the liver is cirrhosed and therefore there is no high increase in only the direct bilirubin levels (but both forms of bilirubin are moderately increased).i am not sure of this but will check out and will let u know as soon as i can.

plz correct me if i am wrong.

Launchpad may be very right. First of all, your question, ARJ, makes all sense... so I looked for urine changes in the features of both PBC and Sclerosing Colangities (for comparison) and nill at Harrison's. Urine is not mentioned at all. Then a bit of googling, and nothing again. Is urine spared (indirect bilirubin-only in these pathologies?) It does seem that way, but it doesn't make sense.shaking head

There are, however, 4 stages to PBC. If launchpad is right, and they are referrign only to cirrhosis (4th stage), there could be direct bilirubin before the 3rd stage (septal)...raised eyebrow
