A 92-year-old nursing home resident with mild Alzheimer’s disease is admitted to the hospital with nonspecific back, chest, and hip pain and generalized weakness. She has no nausea, vomiting, or diarrhea. She has resided in the nursing home for several years. Apart from dementia, her physical examination elicits pain on palpation of her lumbosacral spine. Her motor strength is graded as 3.5/5 in both the upper and lower extremities.
Laboratory studies show the following results: Calcium, 8.0 mg/dL; phosphorus, 2.3 mg/dL; albumin, 3.7 g/dL; globulin, 2.9 g/dL, and alkaline phosphatase, 138 U/L. A chest radiograph shows multiple old rib fractures and osteopenia of the thoracic spine. A radiograph of her hip shows a decrease in bone density with thinning of the cortices and loss of trabeculae, with radiolucent bands noted near the femoral neck.
The most likely diagnosis to explain all of this patient’s abnormalities is:
