Hello bbb,
i am an IMG from Pakistan. I have some questions but let me give you a brief history about my USMLE status.
My step1 score is 83 and i am taking my step2 in jan 2004.. i had planned to take CSA in april2004. I have submited the online form for CSA and have not yet registerd the date. In my country we have to apply for US VISA atleast 3 months befor the intended date of travel and step1 and 2 scores are mandatory before the VISA interview in US embassy. I had planned to take CSA in April. But after the announcement of closing of Philadelphia center in March i dont know what should i do? Ofcourse i shall be assigned a center before i take a date and i wont be ready for CSA in March in case i get PHIl center. now the questions.

First,I want your advice on this matter.

Second, I know it is too early to ask this but i think that i am a bit confused about residency thing. Once we get into a residency, for example IM residency, we are basically residents or interns? after the completion of programme what do we become Consultants or Internists? If i want to go in Cardiology, am i supposed to apply in Internal med or there are programmes for Cardiology. Actually i am asking all this becuse USA system is differnt from ours.

Third, what is the differnce b/w university, community and unive affiliated programmes? what are the benefits of each and which is better?

last.. do you think my step1 score will get me into cardiology prog or a gastroenterology programme.

Long questions i know. I would be glad if you answered.

The questions of when, where, etc are kind of difficult for me to answer as people that I know have done this prior to me knowing them. And visa questions are so hard to me to comment on because of all the INS changes.

Once getting into a residency, you are a resident or houseofficer. The first year of your residency, you are an intern, then you become a junior or senior resident - but the whole program is residency. Now, once you complete, you are an internist. Once you pass the boards, you become a board certified internist.

If i want to go in Cardiology, am i supposed to apply in Internal med or there are programmes for Cardiology.


If you want to become a cardiologist, you first must complete three years of internal medicine residency, followed by a three-four year cardiology fellowship.

University - program that is owned by the University Hospital (Univ of Maryland Med Center, Univ of Pittsburgh Med Center).
Community - stand alone hospital
Univ Affiliated - a community hospital that has an affiliation with a University medical school (no examples here because I can only think of local ones) - say a community hospital that has lots of medical students rotating and doing a lot of their training there.

Which is better? It's going to depend on what you want, who you talk to. I know some univ affiliated hospitals that have bad training programs, and some community hospitals with no direct affiliation that are very good.

Impossible to answer, it's going to depend on too many things (step 2 score, visa status, interviewing skills, in-training exams durings residency, LOR from your PD.

Hard to answer that one.

You become a practicing physician, an Internist. It doesn't give you an MD if you have a different degree - if you are an MBBS, you still are an MBBS.

i have MBBS and MD degree from my country ,NOW on completion of residecncy , i just continue wth my old designation and nothing extra attcahed ?have i got it correct?

If you have earned the MD degree, then you are an MD. You just cannot get the MD from completing a residency in the USA
