The 2006 ECFMG application requires us to submit a "Certification of Identification Form (Form 186)", but I can't find it anywhere on their website. Can somebody please tell me where to get it from?

Yes there are 2 forms. The 187 is the document submission form.
186 is the Certification of Identification Form which is needed to be on file at ECFMG
I have had the same problems as the form is not online in their publications page (everything else is!)

after you complete online application, the last page of application will be this form 186, and you can download it.
