You have successfully treated a 60-yr old man following his 4th experience of major depression. The patient requires weeks of SSRIs before his symptoms were fully relieved. This patient is at highest risk for 1. substance abuse 2. suicide 3. manic episode 4. recurrence of depression 5. medical illness

the question asks after treatment, if it was at the beginning of therapy, that is when patients are at the higher risk for suicide, the anhedonia, and lack of energy are the firsts symptoms to be relieved.
