You start off by applying for the USMLE Step1 or 2 first. From 2004, there will be a slight change in the formatting of the exam - especially the CSA. The TOEFL exam is going to be stopped.
What I can suggest is that you can call up this toll free number and talk to them - 1 800 500 8249. Be patient and stay on the line - they are there from 9-5 pm Eastern time - Mondays to Fridays.
You can also log on to ecfmg.org and update yourself on the latest information.
