Diagnosed by endoscopic Biopsy, PAS Positive macrophages with Gram Positive Bacilli. For Extraintestinal whipple PCR is used In the absence of gastrointestinal symptoms, the diagnosis is often is delayed for several years. The following are extraintestinal Manifstations:- Intermittent low-grade fever occurs in over 50% of cases. Chronic cough is common. There may be generalized lymphadenopathy that resembles sarcoidosis. Myocardial or valvular involvement may lead to congestive failure or valvular regurgitation. Ocular findings include uveitis, vitreitis, keratitis, retinitis, and retinal hemorrhages. Central nervous system involvement in approximately 10% of cases is manifested by a variety of findings such as dementia, lethargy, coma, seizures, myoclonus, or hypothalamic signs. Cranial nerve findings include ophthalmoplegia or nystagmus.

thanx arj so the role of upper gi endoscopy is only in takin biopsy? no characteristic findings will be seen per se during the endoscopy? have you heard about some kind of skin rash associated with whipples?

whipple can virtually involve any organ. I have read of arthritis, not sure about kind of skin lesion

DiagnosticTests: foamy Macrophages in intes.mucosa stain +ve with PAS; Electron microscope- grm+ bacilli Tropheryma whippelli within macrophages (obstructing lymphatic drainage causing fat malabsoprtion - STEATORRHREA) XtraIntestinal features: Any Organ (even skin- pigmentation/rash) rest said by ARJ
