A 55 yr old man have 10 years of poorly controlled HTN..Having dysnea for past 3 days. Last night he slept sitting on a chair. Vital sign is pulse...110/min respiration 30/min BP 120/80 apical impulse felt 2 cm lateral to mid clavicular line in 6 intercostal space.. over the apex, there is an early diastolic low freq sound after S2, this is most likely.. Mid systolic click

s3; the guy has a Left CHF and S3 is actually b/c of LVH.
